Adoption Information Sheet
Through the passage of Act 1399 of 2009, the state can now better
track adoption cases for purposes of trending data and making policy
decisions. Effective January 1, 2010, the attached Adoption
Information Sheet must be completed and submitted to the circuit
clerk prior to the entry of any interlocutory or final decree of
adoption in all adoption cases, including juvenile court cases.
Also on this site you will find a copy of the enacted law as well as
instructions on how to complete and submit your Adoption Information
